Job Description:
The Clinical Pharmacist demonstrates consistent application of knowledge and skills to deliver safe, appropriate, cost effective pharmaceutical care. This position is responsible for patient care as well as the development of effective, professional relationships within the multidisciplinary team.
Intermountain Homecare provides infusion and injectable medications for use at home and clinics throughout Utah, Idaho, and Arizona. Pharmacist's primary responsibility will be verifying proper compounding of IV's and double checking final orders prior to delivery. They will also need to talk to patients, MD's, and RN's to make sure appropriate therapy is arranged.
Job essentials:
Oversee and participate in assuring appropriate medications are prepared and delivered to patients.
Develop pharmacy care plans and monitor patients for appropriate outcomes
Participate in the multidisciplinary team to provide therapeutic recommendations and alternatives, as indicated, to improve patient drug therapy. This may include drug, dosage form, dose, route, dosage interval and duration.
Monitor physician orders and patient labs to ensure safe, correct, and appropriate use of medications.
Provide accurate drug information/education to other healthcare professionals and patients to assure safe and appropriate use of medications.
Serve on committees as a resource on drugs and/or pharmacy policies as required.
Participate in organization surveys (Joint Commission or other applicable regulatory agencies) and audit inspections.
Maintain quality in the department through completion of quality/clinical documentation.
Participate and take responsibility for assuring quality improvement projects are developed, implemented and completed.
Supervise and mentor technicians and other pharmacy staff working within area of responsibility in order to maintain the accuracy and quality of pharmaceutical services.
Actively participate in department cost savings initiatives. Monitor pharmaceutical inventory and formulary and exercise strict inventory control through diligent tracking of all substances
Train and supervise new pharmacy technicians, student-interns, pharmacy students and other supportive staff as needed.
Be knowledgeable and compliant with State and Federal laws, regulations, pharmacy policies, procedures and standard operating procedures, formulary and pertinent Clinical Practice Guidelines.
Provide professional, engaging customer service to all patients, and healthcare providers with whom they work. Other duties as assigned by manager.
Minimum Qualifications
* Current license to practice pharmacy in the state of Utah
* Controlled substance license.
* Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy and the distribution of approved drugs, controlled substances, and investigational agents.
* Effective communication skills
Preferred Qualifications
* Doctor of Pharmacy degree from an accredited college of pharmacy.
Physical Requirements:
Intermountain Homecare Office located at 11520 S Redwood Road South Jordan UT.
Full time 40 hrs/wk. This is a rotating evening shift including every other weekend.
This is a physical job with most hours on your feet. Candidate should be able to lift 50 pounds
